Table 4Exclusion Criteria

Any of the following:

  1. Age <40 y
  2. Not able to be reached by telephone
  3. Not Spanish or English speaking
  4. Not currently a patient at 1 of the 7 participating clinic sites or having no outpatient visits within the past 12 mo
  5. Not planning to continue care at clinic for next 9 mo
  6. Not planning to be in San Francisco area for at least 6 of the next 9 mo, including att ending study
  7. Unable to travel to primary care clinic
  8. Having been identified by PCP as not being capable of participating in the study due to a physical or mental condition

Abbreviation: PCP, primary care provider.
