Successful eculizumab-based desensitization in ABO-incompatible living donor kidney transplantation and its molecular phenotypes

(Submitter supplied) Background: Plasmapheresis/rituximab-based desensitization therapy has successfully reduced anti-ABO antibody levels and suppressed antibody-mediated rejection (AMR) in ABO-incompatible (ABOi) kidney transplantation (KT). However, high titers of anti-ABO antibodies in some patients are refractory to standard desensitization, leading to loss of KT opportunities or AMR. Methods: Eculizumab-based desensitization was used to rescue high-titer ABOi KT patients refractory to plasmapheresis/rituximab-based desensitization. more...

Transciptomic signature of chronic antibody-mediated kidney rejection traited by extracorporeal photopheresis

(Submitter supplied) Background: The benefit of extracorporeal photopheresis on the course of kidney transplant rejection is unknown. The aim of our study was to investigate the variations in transcriptomics on graft biopsies when extracorporeal photopheresis was used to treat chronic humoral rejection after kidney transplantation. Methods: we retrospectively analyzed the mRNA expression of 770 genes of interest in graft biopsies performed before and after treatment. more...

Gene expression data of formalin-fixed-paraffin-embedded (FFPE) renal biopsy tissue assessed with Banff Human Organ Transplant (B-HOT) gene panel assay on NanoString nCounter®

(Submitter supplied) Development and validation of a multi-class molecular model for differential diagnosis of renal allograft rejection, built upon gene expression data from the B-HOT gene panel in a large and diverse cohort of diagnostic FFPE renal biopsies.

Transcriptomic profiling of transplant glomerulopathy to reveal a novel T-cell dominant subclass

(Submitter supplied) Kidney transplant biopsies showing transplant glomerulopathy (cg > 0) and microvascular inflammation (MVI) in the absence of C4d staining and DSAs do not fulfill the criteria for chronic active antibody-mediated rejection (CA-AMR) diagnosis or any other Banff category. In this multicenter intercontinental study including 36 cases, we compared, among other types of data, the transcriptomic profiles of 14 KTx biopsies classified as cg+MVI DSA-/C4d- with 22 classified as CA-AMR DSA+/C4d+ through novel transcriptomic analysis using the NanoString B-HOT panel. more...

Immune and epithelial determinants of age-related risk and alveolar injury in fatal COVID-19

(Submitter supplied) Respiratory failure in COVID-19 is characterized by widespread disruption of the lung’s alveolar gas exchange interface. To elucidate determinants of alveolar lung damage, we performed epithelial and immune cell profiling in lungs from 24 COVID-19 autopsies and 43 uninfected organ donors ages 18-92 years. We found marked loss of type 2 alveolar epithelial (T2AE) cells and increased peri-alveolar lymphocyte cytotoxicity in all fatal COVID-19 cases, even at early stages before typical patterns of acute lung injury are histologically apparent. more...
